I've tryed several times, but it still appearing the same error.
The email is send with sucess, but when i click on the link of the image, appears this error:
Not Found

The requested URL /formulario/PHP/uploadedFiles/sfshpt/foia.jpg was not found on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

When i go see on the ftp, in uploadedFiles folder there is created a new folder but is empty. And the new folder have chmod 755

I really wanna use this form on my website, if anyone can help me?

uploadedFiles > chmod 777

PHP > chmod 755
